They will be providing the eye screening exam at no cost to Service Dogs who qualify (view qualifications). The dates and times of each clinic's participation may vary, you will need to contact the specific clinic you wish to visit below to determine the proper date and time of your appointment. |

| Preview a list of participating board certified veterinary ophthalmologists by State. You may want to check a neighboring state for participating practitioners if none are listed in your state. Once you determine you would like to participate you will need to follow the steps defined below. | ||

| Step 1: In order to receive the free eye examination and Pet Wellness Report you must register for this event. No PIN number will be provided; you do not need a PIN number, but you MUST register to participate. Your information will be collected and provided to the ophthalmologist you will be seeing and to contact your primary care veterinarian to get them prepared for the labs related to the Pet Wellness Report. If you do not register, you cannot participate in the event. | ||
| Step 2: Once you have registered, the web site will push you through to a page that will allow you to search by state to locate a clinic near you. (Clinics may not be available in every state.) | ||
| Step 3: After locating the clinic you would like to visit, call their office directly, identify yourself as participating in the ACVO Service Dog event and make an appointment. Dates and times that this program is offered may vary from clinic to clinic, generally the event is being held the week of May 12th. | ||
| Step 4: Visit the ophthalmologist at the appointed time. Please call ahead if you will be late or have to cancel your appointment. | ||
| Step 5: After your Service Dog's eye examination is complete, your veterinary ophthalmologist will provide you with a copy of your examination form and information. You will need to contact your primary care veterinarian to make an appointment for a blood draw and labs in order to obtain the Pet Wellness Report. The event sponsor, Merial makers of HEARTGARD® Plus (ivermectin/pyrantel) and FRONTLINE®, will have already disseminated lab supplies and information to your veterinarian. The labs and Report will be complimentary to you, compliments of Pet Health Systems, from June 1 - 30, 2008. You must make your appointment within that time period in order to take advantage of the Report. You do not have to participate in this portion of the exam in order to receive the complimentary eye examination, but it is recommended. | ||
