HOW TO FOLLOW-UP ON YOUR PET WELLNESS REPORT EXAM |
On behalf of the American College of Veterinary Ophthalmologists (ACVO) and Pet Health Systems, we would like to thank you for registering and participating in the 2008 Service Dog Eye Exam event. The event is a huge success with well over 1,300 service dogs examined nationwide in an effort to detect any vision and overall body function health screen for related diseases or disorders. As you know, the health and well-being of your service dog is extremely important. Please remember to visit the primary care veterinarian you registered between June 1st and June 30th so that he/she may collect a specimen in order to provide a free blood screen (Chem/CBC/T4) courtesy of Pet Health Systems and Merial Limited, makers of Q1) Can my service dog receive a blood panel if I didn’t register for the event prior to the May 8th deadline? A1) No, only registered service dogs will receive the Pet Wellness Report™. Please sign up for notification of future events. Q2)When/how do I make my appointment with my primary care veterinarian? A2) Please call your veterinarian AFTER June 1st to set up an appointment in June. Your veterinarian is being contacted by Merial and PHS in May to update them regarding the event and to deliver laboratory materials. If you call for an appointment prior to this date your veterinarian may not yet know about the program. Q3) Can I go to any veterinarian to receive the Pet Wellness Report™ for my service dog? A3) No, only the veterinarian you listed during the registration process can collect the specimen and provide the Pet Wellness Report™ for your service dog. Q4) How does my registered veterinarian obtain the blood screen results and how does this process work? A4) Each veterinarian that was listed by the service dog handler/owners during the registration process will receive a blood collection kit and laboratory form with your unique Personal Identification Number (PIN) including you and your dog’s name. There is no cost to you or your veterinarian for the Pet Wellness Report. Once the blood screen is completed and processed, the laboratory results will be faxed to your veterinarian and later uploaded to Pet Health Systems website where you will have access to this information approximately 2 weeks after your visit with your primary care veterinarian. The unique PIN provided on your copy of the laboratory form will give you access and instructions on how to receive your Pet Wellness Report. You and your veterinarian will have access to this data and can discuss the information and schedule a follow-up visit if necessary. As stated in the Pet Wellness Report, the report is designed to be educational and help you become a better informed dog owner. The report cannot replace the advice of your veterinarian or assist you in making a health decision on your animal. Please consult with your veterinarian for confirmation of any concerns and needed treatment. All follow-up visits and other diagnostic requirements including any tests, will not be covered or paid, in whole or in part, by this program. Only the eye exam, blood screen and Pet Wellness Report™ are provided at no cost through the generosity of the ACVO, Pet Health Systems and Merial Limited. Q5) Can I bring multiple dogs to my primary care veterinarian for a Pet Wellness Report™? A5) Yes, as long as they are service dogs that were registered for the event and that veterinarian’s information was provided per dog during the registration process. Q6) Is there still time to register? A6) No, the registration period is closed and the eye exam portion of the event has ended. We would encourage you to register for notification of future events. If you know of someone else who would qualify for this event and would like to register, please ask them to sign up so we may determine need and also notify them when that occurs. |
